Job Summary
Occupational Therapist needed for home-based, per diem OT in Queens, NY, delivering 1:1 home and community-based therapy. Responsibilities include evaluating patients, developing individualized treatment plans based on physician orders, delivering OT to maintain/improve function, prescribing or advising on interventions, educating family members and home health aides, documenting initial exams and ongoing progress, and traveling to patients’ homes within the assigned geographic area. Opportunity for 1099 or W-2 engagement, flexible scheduling (weekdays and/or weekends), and mentorship for new graduates. Must be a licensed NY OT (or eligible) with ability to travel to patients’ homes; geriatric care experience preferred.
Required Qualifications
- Occupational Therapist Program Graduate
- Licensed Occupational Therapist in the state of New York or eligible
- Ability to travel to patients’ homes within the assigned geographic territory
- Home health experience preferred but not required (mentorship provided)
- Strong communication skills with patients, caregivers, and management

- Must be willing to treat geriatric patients
