Job Summary
Conduct home health visits to adult and geriatric patients in Fort Wayne, Indiana, performing thorough evaluations of functional status, muscle function, and cognitive aspects to develop individualized intervention plans. Assess home environments for hazards, identify equipment needs, and revise care plans in collaboration with nursing staff. Educate patients and family members on rehabilitative care and safety while accurately documenting observations and interventions using a touch pad tablet. Requires a current Indiana Occupational Therapist license, one year of recent experience, and proficiency with electronic medical records.
Required Qualifications
- A current Indiana Occupational Therapist license
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Organizational skills
- Problem solving skills
- Solid computer skills
Desired Qualifications
