Occupational Therapist- Home Health Visits

Job Summary
Conduct home health visits to adult and geriatric clients in Schyulkill County, performing thorough evaluations of functional status, muscle function, and cognitive performance. Assess home environments for hazards, identify equipment needs, and develop individualized intervention plans to promote independent living. Continually revise care plans, collaborate with nursing staff, and educate patients and families on rehabilitative activities. Accurately document all observations and interventions using a touch pad tablet. Requires a current Pennsylvania Occupational Therapist license, one year of recent OT experience, and strong interpersonal skills.
Required Qualifications
- A current Pennsylvania Occupational Therapist license
- A minimum of one year of recent work experience as an Occupational Therapist
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills

- Solid computer skills
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
