Occupational Therapist – Home Health (Relocation/Hiring Incentives Available)

Conduct one-on-one care for patients in their homes, helping them regain independence by examining, evaluating, and testing individuals with mechanical, physiological, and developmental impairments. Design, implement, and modify treatment interventions to alleviate functional limitations while reducing injury risk and promoting fitness across populations of all ages. Collaborate with interdisciplinary teams to create individualized plans, administer care, and engage in consultation, education, and research to restore function and enhance quality of life. Maintain a productivity model based on a 30-point weekly expectation using the EPIC EMR system.
