Occupational Therapist Home Health-PRN

Provide home health occupational therapy services to patients in their homes. Assess daily living and work-related skills, develop retraining programs with measurable objectives, and administer/provide interpretation of functional tests. Collaborate with physicians and rehabilitation team to develop and revise Plan of Care, deliver therapy to improve strength, coordination, range of motion, and functional independence, and teach compensation strategies. Review and expand retraining programs to align with behavioral goals and rehabilitation guidelines. Consult with vocational team members to implement vocationally oriented plans; report patient status to physicians, rehabilitation staff, clinical manager, patients and families. Participate in care coordination and discharge planning; design community reintegration activities; recommend adaptive equipment and educate staff/family to implement retraining programs. Supervise and evaluate Occupational Therapy Assistant and Home Health Aide performance; document care observations, interventions, and discharge summaries; ensure orders are implemented. Drive to patient homes as required and use skills to impact patient outcomes. Required qualifications include current OT licensure, CPR, and valid driver’s license with vehicle insurance; preferred qualifications include six months of OT experience and Home Health experience.
