Job Summary
Occupational Therapist in home health responsible for assessing daily living/work-related skills, developing therapeutic retraining programs with measurable objectives, administering function tests, and coordinating with physicians and the care team to support rehabilitation and independence. Delivers therapeutic treatment to improve/restore strength, coordination, range of motion, and function, designs and expands retraining programs, consults with vocational and interdisciplinary team members, documents patient status, participates in care coordination and discharge planning, and may supervise OT staff and trainees. Requires OT licensure, CPR, and a valid driver’s license with reliable transportation; TB screening participation may be required; role is patient-facing in home settings and aligns with Humana/CenterWell Home Health standards.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver’s license, auto insurance and reliable transportation.
