Job Summary
Occupational Therapist (Home Health) to deliver in-home rehabilitative therapy, assess daily living/work-related skills, develop retraining plans, interpret function tests, collaborate with physicians and the interdisciplinary team, supervise OTAs and Home Health Aides, document patient progress, and coordinate care and discharge planning. Role emphasizes strengthening, range of motion, functional restoration, adaptive equipment design, family/staff training, and community reintegration within a Humana CenterWell Home Health setting. Licensure, CPR, and reliable transportation are required; preferred experience in Home Health and minimum six months of OT experience.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver‘s license, auto insurance and reliable transportation.
