Job Summary
Provides quality rehabilitative occupational therapy to home health clients; participates in development of occupational therapy plans of care; makes visits for assessment and evaluations and communicates with physician before start of care as needed; completes OASIS assessments; implements plans of care; educates and instructs team members, patients, and families; evaluates outcomes of care; supervises paraprofessionals as needed; participates in discharge planning and interdisciplinary team meetings; ensures documentation in EMR; adheres to infection control standards and safety procedures; collaborates with service providers and adheres to agency policies; maintains confidentiality and follows regulatory requirements; requires Bachelor of Science in Occupational Therapy and active OT license; minimum 1 year of experience preferred.
Required Qualifications
- Bachelor of Science in Occupational Therapy required
- Active and unencumbered Occupational Therapist license in state(s) of employment required
- At least one (1) year of experience as an Occupational Therapist in an acute care, home health, or rehabilitation setting highly preferred
