Occupational Therapist Home Health-PRN

Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes to evaluate function, confer with physicians to revise Plans of Care, and provide instruction to improve strength, coordination, and range-of-motion. Review retraining programs, consult on vocational plans, and design community reintegration activities. Recommend adaptive equipment, train staff and families on self-care skills, and supervise Occupational Therapy Assistants. Document care observations, implement interim orders, and submit evaluations and discharge summaries. Report patient status to the physician and interdisciplinary team.
