Job Summary
Assess patients' daily living skills, administer diagnostic tests, and develop therapeutic retraining programs with measurable objectives. Confer with physicians to revise Plans of Care, provide instruction to restore strength and function, and design community reintegration activities. Supervise Occupational Therapy Assistants and Home Health Aides, document care observations and interventions, and coordinate discharge planning. Report patient status to the physician and interdisciplinary team. This role supports patients managing chronic conditions or recovering from injury at CenterWell Home Health, a Humana company.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
