Job Summary
Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes to evaluate function, confer with physicians to revise Plans of Care, and provide instruction to restore strength and coordination. Implement vocationally oriented plans, design community reintegration activities, and recommend adaptive equipment for residence or work environments. Supervise Occupational Therapy Assistants and Home Health Aides, train family members on hygiene and dressing skills, and coordinate discharge planning. Accurately document all care observations, interventions, and evaluations while reporting patient status to the Clinical Manager.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
- Personal vehicle liability insurance in accordance with state minimums or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
