Job Summary
Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes to evaluate function, then provide treatment and instruction to restore strength, coordination, and range of motion. Confer with physicians to revise Plans of Care, design community reintegration activities, and recommend adaptive equipment. Supervise OT Assistants and HHA performance, document care observations, and submit evaluation reports and discharge summaries. Participate in care coordination, discharge planning, and interdisciplinary team meetings to support patient recovery and independence.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (patient-facing role)
- Valid state driver's license
- Personal vehicle liability insurance
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
