Job Summary
Assess and screen patients' daily living skills to develop therapeutic retraining programs, administer diagnostic tests, and interpret function levels to assist physicians. Confer with the healthcare team to revise the Plan of Care, provide treatment and instruction to restore strength and coordination, and design community reintegration activities. Consult with vocational team members to implement vocationally oriented plans and recommend adaptive equipment for improved living environments. Supervise Occupational Therapy Assistants and Home Health Aides, train staff and family members on hygiene skills, and document all care observations and interventions. Report patient status to the physician and care management staff, participate in discharge planning, and submit evaluation plans and discharge summaries.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
- Maintain personal vehicle liability insurance in accordance with state minimums or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
