Job Summary
Assess patients' daily living skills, administer diagnostic tests, and develop therapeutic retraining programs with measurable objectives. Confer with physicians to revise Plans of Care, provide treatment and instruction to restore function, and design community reintegration activities. Supervise Occupational Therapy Assistants, document care observations and interventions, and coordinate discharge planning. Recommend adaptive equipment and train staff on hygiene and grooming skills. Report patient status to the clinical team and family members. This role supports CenterWell Home Health in helping patients regain independence through personalized rehabilitation.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
