Job Summary
Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in home settings, confer with physicians to revise Plans of Care, and provide instruction to restore strength and function. Review retraining programs, consult with vocational team members, and design community reintegration activities. Recommend adaptive equipment, train family members on hygiene skills, and supervise Occupational Therapy Assistants. Document all care observations, interventions, and evaluations accurately while reporting progress to the physician and care management staff.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
