Job Summary
Assess patient daily living skills, administer diagnostic tests, and develop therapeutic retraining programs with measurable objectives. Confer with physicians to revise Plans of Care, provide treatment and instruction to restore function, and design community reintegration activities. Consult with vocational team members to implement vocationally oriented plans and recommend adaptive equipment for patient environments. Supervise Occupational Therapy Assistants and Home Health Aides, document care observations and evaluations, and coordinate discharge planning. Report patient progress to physicians and care management staff. Requires current OT licensure, CPR certification, and valid driver's license.
Required Qualifications
- Current and unrestricted OT licensure

- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
