Job Summary
Assess patients' daily living skills, administer diagnostic tests, and develop therapeutic retraining programs with measurable objectives. Confer with physicians to revise Plans of Care, provide instruction to restore function, and design community reintegration activities. Supervise OTAs and HHAs, train family members on self-care skills, and recommend adaptive equipment. Document all care observations, interventions, and evaluations; submit discharge summaries to care management. Coordinate discharge planning and report patient progress to the interdisciplinary team.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
