Job Summary
Assess patient daily living and work-related skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes to evaluate function, confer with physicians to revise treatment plans, and provide instruction to improve strength, coordination, and range-of-motion. Review retraining programs against behavioral goals, consult with vocational team members, and design community reintegration activities. Recommend adaptive equipment for residence or work environments and train staff and family members on dressing, feeding, and hygiene skills. Supervise Occupational Therapy Assistants and Home Health Aides, document care observations and interim orders, and submit evaluation plans and discharge summaries. Report patient status and progress to the physician and care management staff.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
- Maintain personal vehicle liability insurance in accordance with state minimums or specified limits ($25,000 bodily injury per person/event, $10,000 property damage)
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
