Occupational Therapist Home Health-PRN

Assess patient daily living and work-related skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic and prognostic tests in home settings, confer with physicians to revise the Plan of Care, and provide treatment instruction to restore strength, coordination, and function. Review retraining programs, consult with vocational team members, and design community reintegration activities. Recommend adaptive equipment, train staff and family members on self-care skills, and supervise Occupational Therapy Assistants. Document patient observations, interventions, and evaluations accurately, reporting status to the physician and care management staff. Participate in discharge planning and interdisciplinary meetings to coordinate patient needs.
