Job Summary
Assess and screen patient daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes to evaluate function, confer with physicians to revise treatment plans, and provide instruction to restore strength and coordination. Review retraining programs, consult with vocational teams, and design community reintegration activities. Recommend adaptive equipment, train staff and families on hygiene skills, and supervise Occupational Therapy Assistants. Document patient observations, interventions, and evaluations thoroughly, submitting discharge summaries to care management.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
- Maintain personal vehicle liability insurance in accordance with state minimums or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
