Job Summary
Occupational Therapist Home Health PRN providing in-home rehabilitation to improve patients' strength, mobility and independence. Responsibilities include assessing daily living/work-related skills, developing therapeutic retraining programs with measurable objectives, administering and interpreting function tests, coordinating with physicians and care teams, delivering therapeutic treatment to improve/restore strength, range of motion and function, planning care/discharge coordination, designing adaptive equipment, and supervising OT assistants and Home Health Aides. Emphasis on designing community reintegration activities, documenting care, and collaborating with interdisciplinary team to support patient independence and quality of life.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver''s license, auto insurance and reliable transportation
