Occupational Therapist Home Health-PRN

Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in home settings, confer with physicians to revise Plans of Care, and provide instruction to restore strength and function. Consult with vocational team members to implement reintegration plans and design adaptive equipment for patient environments. Supervise Occupational Therapy Assistants and train family members on hygiene and grooming skills. Document patient observations, interim orders, and discharge summaries accurately. Submit evaluation plans and progress reports to clinical management and patient families.
