Occupational Therapist Home Health PRN

Occupational Therapist Home Health PRN role delivering in-home rehabilitation and functional retraining to help patients regain strength, mobility and independence. Responsibilities include assessing daily living/work-related skills, developing Plan of Care with therapeutic objectives, administering and interpreting function tests, coordinating with physicians and care team, providing hands-on therapy to improve strength, range of motion, coordination and functional performance, training patients and family members, ensuring thorough documentation, and participating in discharge planning and community reintegration activities. Requires current OT licensure, CPR, valid driver's license and reliable transportation. Pay range is $85,400–$117,500 per year; per-visit compensation is included with full-time equivalent considerations. TB screening and driver-safety program participation are part of the role.
