Job Summary
Provide occupational therapy in patients' homes, assess daily living/work-related skills, and develop therapeutic retraining programs with measurable objectives. Administer and interpret functional tests to assist physicians in evaluating the patient’s level of function. Confer with physicians and the rehab team to participate in plan-of-care development. Deliver therapeutic treatment per physician orders to improve/restore strength, coordination, range of motion, and functional independence; teach compensation strategies. Review and expand retraining programs to align with behavioral goals and rehab program guidelines. Collaborate with vocational team members to develop vocationally oriented plans. Document patient status and progress to the physician, rehab staff, Clinical Manager, patient, and family. Participate in care coordination and discharge planning; design community reintegration activities and adaptive equipment to enhance home/work environments. Train staff and family to carry out retraining programs and supervise OT Assistant and Home Health Aide performance. Ensure thorough documentation of care observations and evaluations. Maintain focus on patient impact and interprofessional collaboration.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver’s license, auto insurance and reliable transportation.
