Job Summary
PRN Occupational Therapist - Home Health in the DeKalb, IL area. Assess and screen patients’ daily living/work-related skills and develop therapeutic retraining programs with measurable objectives. Administer and interpret function tests to assist physicians in evaluating function. Partake in Plan of Care development/revision, provide in-home therapeutic treatment to improve strength, range of motion, coordination, and functional performance. Design and expand retraining programs aligned with patient goals; consult with vocational/health-team members to implement vocationally oriented plans; report patient status to physicians and care teams; participate in care coordination and discharge planning; design community reintegration activities and adaptive equipment recommendations; train staff and family members to carry out retraining programs; supervise Occupational Therapy Assistant and Home Health Aide performance; accurately document care observations and plans. Role requires OT licensure, CPR certification, and valid driver’s license with reliable transportation. Preferred: minimum six months of OT experience and Home Health experience. Pay per visit and annual base pay ranges are provided in the posting; scheduling indicates full-time equivalence may vary by geographic location. The role includes TB screening and driver-safety program participation as part of Humana/CenterWell Home Health policies.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver‘s license, auto insurance and reliable transportation.
