Job Summary
Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes to evaluate function, provide treatment and instruction to restore strength and coordination, and design community reintegration activities. Confer with physicians to revise Plans of Care, coordinate discharge planning, and recommend adaptive equipment. Supervise Occupational Therapy Assistants and document all care observations, interventions, and evaluations. Submit treatment plans and discharge summaries to supervisors and care management staff.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
