Occupational Therapist Home Health-PRN

In-home occupational therapy with CenterWell Home Health involves assessing patients’ daily living/work-related skills, developing therapeutic retraining plans with measurable goals, and delivering therapy in patients’ homes or other settings per physician orders. The OT will collaborate with physicians and the care team to revise the Plan of Care, provide instruction to patients and families, supervise staff, document care thoroughly, participate in care coordination and discharge planning, and design community reintegration activities and adaptive equipment to improve independence and participation.
