Occupational Therapist - Home Health - Per Diem

Conduct medically prescribed Occupational Therapy services, including evaluations, treatment implementation, and home safety assessments, directly under director supervision. Plan individualized therapy programs for patients and collaborate with the Home Health team to ensure services meet patient needs. Evaluate and document progress using EMR OASIS systems while facilitating durable medical equipment ordering and training. Interpret departmental policies for patients, caregivers, and families, and participate in peer feedback and quality improvement processes. Perform hospice evaluations and treatment as ordered, maintaining fiscal responsibility and targeted key performance indicators.
