Occupational Therapist Home Health-Per Diem

Assess patients' daily living skills and develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests to evaluate patient function and provide treatment to restore strength, coordination, and range of motion. Confer with physicians to revise Plans of Care, coordinate discharge planning, and design community reintegration activities. Supervise Occupational Therapy Assistants and train family members on hygiene and dressing skills. Document all care observations, implement interim orders, and submit evaluation plans to management. Report patient status to the clinical team and recommend adaptive equipment to improve home environments.
