Job Summary
Assess patients' functional abilities and develop individualized treatment plans centered on activities of daily living. Implement rehabilitation techniques for complex neurologic injuries and teach adaptive strategies, assistive device use, and home modifications. Provide skilled occupational therapy interventions in the home environment while collaborating with nurses, physical therapists, and physicians. Document care, progress, and outcomes in compliance with regulatory and agency standards. Travel to patient homes within the designated coverage area. Master's degree in Occupational Therapy with active licensure in Minnesota and two years of independent clinical experience required. Valid driver's license with ability to travel across all service locations necessary.
Required Qualifications

- Active OT licensure or certification in Minnesota
