Job Summary
Conduct home health therapy interventions and establish individualized treatment plans for patients. Complete comprehensive clinical evaluations to ensure quality standards and address assessment areas requiring attention. Develop discharge plans and communicate them to appropriate parties while completing accurate clinical documentation within established deadlines. Supervise assigned assistants and perform other job-related duties as assigned. Requires Master's or Doctor of Occupational Therapy degree, state licensure, CPR/BLS certification, and a valid driver's license. Full-time day shift position in Yuba City, CA.
Required Qualifications
- Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD)
- Occupational Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Access to and understanding of E-Verify Participation and Right to Work notices
