Job Summary
Conduct home health therapy interventions using appropriate protocols to restore physical, social, and mental functions. Complete patient evaluations, establish individualized treatment and discharge plans, and ensure plans are communicated to all parties. Supervise assigned assistants and perform other job-related duties as assigned. Deliver comprehensive clinical documentation within established deadlines while maintaining quality standards. Requires Master's degree in Occupational Therapy, state licensure, CPR/BLS certification, and a valid driver's license. Based in Gridley, CA, this full-time day shift role offers Adventist Health's personalized in-home care across California, Hawaii, Oregon, and Washington.
Required Qualifications
- Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD)
- Occupational Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Access to and understanding of E-Verify Participation and Right to Work notices
