Job Summary
Conduct home health and hospice therapy interventions using clinical techniques to restore physical, social, and mental functions. Plan and lead treatment programs while serving as a co-leader in interdisciplinary patient groups. Complete patient evaluations to ensure quality standards, establish individualized care and discharge plans, and communicate these plans to appropriate parties. Supervise assigned assistants and perform other job-related duties as assigned. Maintain comprehensive and accurate clinical documentation within established deadlines. Requires Master's or Doctor of Occupational Therapy degree, state licensure, CPR/BLS certification, and valid driver's license. Full-time day shift (8 hours) in Lincoln, CA.
Required Qualifications
- Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD)
- Occupational Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
