Job Summary
Occupational Therapist (OT) for Home Health providing in-home therapy across the care continuum. Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while serving as co-leader in an interdisciplinary team. Responsibilities include completing evaluations, establishing individualized treatment and discharge plans, delivering therapy interventions using approved protocols, maintaining clinical documentation, and supervising assigned assistants. Requires a Master’s degree in Occupational Therapy (or Doctor of OT as allowed), active OT licensure in applicable states, CPR/BLS certification, and a valid driver’s license (must be 21+). Vaccinations and E-Verify participation are part of organizational requirements. Day shift, full-time position with Adventist Health network.

- Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD): Required
- Occupational Therapy licensure in all applicable states of practice: Required
- CPR or BLS certification: Required
- Valid Driver's License and age 21+: Required
