Job Summary
Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups. Completes evaluations to ensure quality standards, establishes individualized treatment plans, provides therapy interventions using appropriate protocols, creates discharge plans, and ensures documentation is completed accurately and within deadlines. Supervises assigned assistants. Requires Master's degree in OT (or OTD) and active state licensure; CPR/BLS; and a valid driver's license (21+). Must meet vaccination requirements as a condition of employment and comply with E-Verify. Home health/hospice services across CA, HI, OR, and WA.
Required Qualifications
- Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD) required; only if obtained license on or after January 2007; or foreign equivalent
- Occupational Therapy licensure in all applicable states of practice: Required
- CPR or BLS certification from AH policy: Required
- Valid Driver's License and must be at least 21 years of age or older: Required
