Job Summary
Conduct home health and hospice treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and communicate these plans to appropriate parties. Provide therapy interventions using established protocols, supervise assigned assistants, and maintain comprehensive clinical documentation within deadlines. Requires Master's or Doctorate in Occupational Therapy, state licensure, CPR/BLS certification, and a valid driver's license. Full-time day shift (8 hours) in Colusa, CA. Required Qualifications
- Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD)
- Occupational Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
