Job Summary
Conduct home health and hospice therapy interventions using appropriate clinical techniques to restore physical, social, and mental functions. Plan and lead interdisciplinary treatment programs, establishing individualized care and discharge plans while communicating them to appropriate parties. Complete comprehensive clinical evaluations, documentation, and assessments to ensure quality standards are met. Supervise assigned assistants and perform other job-related duties as assigned. This role requires an OT license, Master's or Doctorate degree, CPR/BLS certification, and a valid driver's license. Located in Happy Valley, OR, Adventist Health offers personalized in-home care across California, Hawaii, Oregon, and Washington.
Required Qualifications
- Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD)
- Occupational Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- License obtained on or after January 2007
- Foreign equivalent to the Master's degree or OTD
- All required vaccinations as a condition of employment
- Acknowledgement of E-Verify Participation and Right to Work notices
