Job Summary
Conduct home health and hospice treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and perform therapy interventions using appropriate protocols. Supervise assigned assistants and maintain comprehensive clinical documentation within established deadlines. Requires Master's degree in Occupational Therapy, state licensure, CPR/BLS certification, and a valid driver's license. Full-time role based in Troutdale, OR.
Required Qualifications
- Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD)
- Occupational Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- License obtained on or after January 2007
- Foreign equivalent to the Master's degree or OTD
- All required vaccinations as a condition of employment
- Acknowledgement of E-Verify Participation and Right to Work notices
