Job Summary
Conduct home health and hospice therapy interventions, complete patient evaluations, and establish individualized treatment and discharge plans. Supervise assigned assistants and ensure accurate clinical documentation is completed within established deadlines. Serve as a co-leader in interdisciplinary patient groups to restore physical, social, and mental functions while meeting department objectives. Requires Master's degree in Occupational Therapy, state licensure, and CPR certification. Located in Pacific City, OR.
Required Qualifications
- Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD)
- Occupational Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- All required vaccinations as a condition of employment
- E-Verify participation acknowledgment
