Occupational Therapist Home Health

Assess patient daily living and work-related skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic and prognostic tests to evaluate patient function, then confer with physicians and care teams to revise treatment plans. Provide therapeutic instruction to restore strength, coordination, and range-of-motion while teaching compensation measures and adaptive equipment use. Supervise Occupational Therapy Assistants and Home Health Aides, document care observations and interventions, and coordinate discharge planning activities. Report patient status and progress to the Clinical Manager and family members.
