Job Summary

Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes, confer with physicians to revise Plans of Care, and provide instruction to restore strength and function. Consult with vocational team members to implement reintegration plans and recommend adaptive equipment for residence or work environments. Supervise Occupational Therapy Assistants and Home Health Aides, while documenting care observations, interventions, and discharge summaries. Participate in care coordination and discharge planning, and train staff and families on hygiene and grooming skills.

Required Qualifications

  • Current and unrestricted OT licensure
  • Current CPR certification
  • Valid driver's license
  • Auto insurance
  • Reliable transportation
  • TB screening
  • Maintain personal vehicle liability insurance in accordance with state minimum required limits or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher

Desired Qualifications

  • Minimum six months of occupational therapy experience
  • Home Health experience
