Job Summary
Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes, confer with physicians to revise Plans of Care, and provide instruction to improve strength, coordination, and function. Review retraining programs, consult with vocational team members, and design community reintegration activities. Recommend adaptive equipment, train staff and families on hygiene skills, and supervise Occupational Therapy Assistants. Document patient observations, interventions, and evaluations daily, reporting progress to the physician and care management staff.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (patient-facing role)
- State driver's license maintenance
- Personal vehicle liability insurance in accordance with residing state minimum required limits or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage or whichever is higher
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
