Job Summary
Conduct home health and hospice therapy interventions using appropriate clinical techniques to restore physical, social, and mental functions. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and communicate these plans to appropriate parties. Supervise assigned assistants and perform other job-related duties as assigned. This role is based in Bay City, OR, with Adventist Health offering personalized in-home care across California, Hawaii, Oregon, and Washington.
Required Qualifications
- Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD)
- Occupational Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- License obtained on or after January 2007
- Foreign equivalent to the Master's degree or OTD
- All required vaccinations
- E-Verify participation acknowledgment
