Job Summary
Assess patient daily living skills, administer diagnostic tests, and develop therapeutic retraining programs with measurable objectives. Confer with physicians to revise treatment plans, provide instruction to improve strength and range of motion, and design community reintegration activities. Supervise therapy assistants and document all care observations, interventions, and evaluation reports. Recommend adaptive equipment and train family members on hygiene and grooming skills. Report patient progress to the clinical team and participate in discharge planning. This role supports patients recovering from injury or managing chronic conditions within a comprehensive home care team.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
