Occupational Therapist (Home Health)

Job Summary
Conduct segments of treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and perform therapy interventions using appropriate clinical protocols. Supervise assigned assistants and ensure all clinical documentation is comprehensive and accurate within established deadlines. Maintain current Occupational Therapy licensure, CPR/BLS certification, and a valid driver's license. Work in our home health and hospice agencies serving California, Hawaii, Oregon, and Washington, with locations including Forest Grove, OR.
Required Qualifications
- Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD)
- Occupational Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- License obtained on or after January 2007
- Foreign equivalent to the Master's degree or OTD
- All required vaccinations
- E-Verify participation acknowledgment
