Occupational Therapist Home Health

Assess patient daily living and work-related skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic and prognostic tests in homes or other settings to evaluate patient function. Confer with physicians and care team members to develop and revise the Plan of Care Treatment. Provide therapeutic treatment and instruction to improve strength, coordination, range-of-motion, and function, or teach compensation measures. Review and expand retraining programs consistent with behavioral goals and rehabilitation guidelines. Consult with vocational team members to implement vocationally oriented plans. Report patient status and progress to the physician, staff, and families. Participate in care coordination, discharge planning, and community reintegration activities. Recommend or design special adaptive equipment for residences and working environments. Train staff and family members on retraining programs including dressing, feeding, grooming, and hygiene skills. Supervise, instruct, and evaluate Occupational Therapy Assistants and Home Health Aides. Accurately document care observations, interventions, and evaluations on the day services are rendered. Submit evaluation, treatment plans, and discharge summaries to supervisors and care management staff.
