Occupational Therapist Home Health

Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes to evaluate function, confer with physicians to revise Plans of Care, and provide instruction to restore strength and coordination. Design community reintegration activities, recommend adaptive equipment, and train staff or family members on hygiene and dressing skills. Supervise Occupational Therapy Assistants and Home Health Aides, document care observations and interventions, and submit evaluation reports to supervisors. Submit discharge summaries to care management staff. This role supports patients managing chronic conditions or recovering from injury, working within CenterWell's interdisciplinary team to help individuals regain independence.
