Job Summary
Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes, confer with physicians to revise Plans of Care, and provide treatment to restore strength, coordination, and function. Consult with vocational team members to implement reintegration plans, design adaptive equipment, and train staff on hygiene and grooming skills. Supervise Occupational Therapy Assistants and Home Health Aides, while accurately documenting all care observations, interventions, and evaluations. Submit discharge summaries to care management and coordinate discharge planning activities.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
