Job Summary
Assess patient daily living skills and develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in homes to evaluate function, confer with physicians to revise treatment plans, and provide instruction to restore strength and coordination. Implement vocationally oriented plans, design community reintegration activities, and recommend adaptive equipment for residence or work environments. Supervise Occupational Therapy Assistants and Home Health Aides, document care observations and interim orders, and coordinate discharge planning with interdisciplinary teams. This role supports patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization within CenterWell Home Health's personalized care framework.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
