Job Summary
Assess and screen patients' daily living skills to develop therapeutic retraining programs with measurable objectives. Administer diagnostic tests in home settings, confer with physicians to revise Plans of Care, and provide instruction to improve strength, coordination, and range-of-motion. Review retraining programs, consult on vocational plans, and design community reintegration activities. Recommend adaptive equipment, train staff and families on self-care skills, and supervise Occupational Therapy Assistants. Document care observations, interventions, and evaluations; submit discharge summaries and interim orders. Report patient status to the physician and care management staff.
Required Qualifications
- Current and unrestricted OT licensure
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility
Desired Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
